Telehealth Informed Consent

By completing the RxVigor intake assessment and proceeding with a telehealth consultation, you acknowledge that you have read, understood, and agree to the terms of this Telehealth Informed Consent.

1. What Is Telehealth?

Telehealth involves the delivery of healthcare services using electronic communications, information technology, or other means between a healthcare provider and a patient who are not in the same physical location. RxVigor uses asynchronous telehealth, meaning your physician reviews your submitted health information and medical history to make clinical decisions without a real-time video or phone call.

2. Benefits of Telehealth

  • Improved access to medical care by enabling patients to remain at home or in a convenient location
  • Efficient medical evaluation and management
  • Obtaining relevant information from specialists without in-person visits
  • Reduced travel time and associated costs

3. Risks and Limitations of Telehealth

As with any medical service, there are potential risks associated with telehealth. These include, but are not limited to:

  • Information Transmission: Delays in medical evaluation and treatment could occur due to deficiencies or failures of the equipment or technology.
  • Incomplete Information: In rare cases, a lack of access to complete medical records may result in adverse drug interactions or allergic reactions or other judgment errors.
  • Privacy: Despite reasonable security measures, telehealth involves the transmission of medical information digitally, which carries inherent privacy risks.
  • No Physical Examination: The physician reviewing your case cannot perform a physical examination. You may be asked to seek in-person care if your condition requires it.
  • Technology Failures: Technical failures may interrupt or prevent telehealth services.

4. Patient Rights and Responsibilities

You have the right to:

  • Withhold or withdraw consent for telehealth services at any time without affecting your right to future care
  • Inspect all information obtained and recorded in the course of a telehealth interaction
  • Receive a copy of all documentation provided to or by the consulting provider
  • Expect that all laws regarding patient privacy and confidentiality shall be observed

You are responsible for:

  • Providing accurate and complete health information, including current medications, allergies, and medical history
  • Informing your RxVigor physician of any changes in your health status
  • Following your physician's instructions and recommendations
  • Seeking emergency care immediately if you experience a medical emergency

5. Scope of Services

RxVigor physicians provide asynchronous telehealth consultations for GLP-1 weight loss therapy (semaglutide and tirzepatide) and PCOS management. Our services are not intended to replace your primary care physician. We do not provide emergency medical services. If you are experiencing a medical emergency, call 911 immediately.

RxVigor physicians are licensed in the state(s) in which they practice. Telehealth services are only available to patients located in states where our physicians are licensed to practice.

6. Confidentiality

All information disclosed in the course of your telehealth consultation is confidential and will not be disclosed to third parties except as authorized by you or as required by law. Your health information is protected under HIPAA. Please review our HIPAA Notice of Privacy Practices for full details.

7. Prescription Policy

Prescriptions are issued solely at the discretion of the reviewing physician based on your health assessment. Not all patients will qualify for GLP-1 therapy. A prescription does not guarantee approval; the physician may decline to prescribe if they determine it is not clinically appropriate for you. Prescriptions are sent directly to our licensed compounding pharmacy partners.

8. Follow-Up Care

Your RxVigor care team will conduct monthly check-ins to monitor your progress and adjust your treatment plan as needed. You are encouraged to report any side effects, concerns, or changes in your health status promptly through the patient portal or by emailing [email protected].

9. Consent

By proceeding with the RxVigor intake assessment, you confirm that:

  • You have read and understood this Telehealth Informed Consent
  • You voluntarily consent to receive telehealth services from RxVigor
  • You understand the benefits, risks, and limitations described above
  • You are 18 years of age or older
  • The information you provide in your health assessment is accurate and complete to the best of your knowledge
